To first-time visitors
1. Please fill out the medical treatment application form and submit it to the reception desk together with the following documents.
Items to bring
●Residence card or passport
●Health insurance certificate (student, long-term stay)
●For holders of the following items: Letter of introduction, certificate of receipt of public funds, medical record handbook (Letters of recommendation can be accepted in Japanese or English)
*Please understand that consultation will be refused if your status of residence cannot be confirmed.
*To verify identity by photo ID card, and prevent serious accidents due to patient misidentification
*To comply with reporting requirements (patient nationality, language used, etc.) as an accredited medical institution accepting foreign patients
*To comply with the request from Tokyo Regional Immigration Bureau to confirm the Certificate of Eligibility
*Treatment will be provided in Japanese or through machine translation.
*Services cannot be covered if you do not have a health insurance certificate and will be paid in full at your expense.
*Please be sure to inform the reception desk in the case of consultation not covered by insurance (accident, occupational accident, vaccination, health checkup, etc).
Adoption of ili offline interpretation device
Our hospital has adopted the ili offline interpretation device to accommodate persons not able to speak Japanese. The use of ili allows staff to communicate in Japanese, Chinese, or English.
To visitors for reexamination
1. Please insert your consultation ticket into the automated reception machine and select the department for examination.
2. Please take the consultation slip issued by the reception machine, and submit your consultation ticket and health insurance certificate to the reception desk.
If you are unsure of how to operate the machine, please ask the reception desk staff.

*Be sure to bring your health insurance certificate with you every time. Be sure to notify the reception desk if your health insurance certificate has changed.
*Persons who have not undergone consultation for some time may not be able to perform reception using the reception machine. Also, contact the reception desk staff concerning consultations different from the usual.
About the issuance of medical certificates, letters of introduction, and other documents in English
●The fee for a standard medical certificate will in principle be 6,000 yen (tax excluded). Please inquire at the window regarding the issuance of other documents.
●Please understand that time may be required for translation of some content.
●Documents cannot be issued in foreign languages other than English.

Access by public transportation
Get out at Neginohashi, proceed in the direction of the bus route, and turn left at the first intersection (Sasame Higashi-sho (Minami)).

Access by car
Proceed along Chuo-dori Avenue, cross the Sasame-gawa River, and turn left at the Sasame Higashi-sho (Minami) intersection.
*Two parking lots with 35 spaces total are adjacent to and across from the hospital.
